Math
It has always been important to me that my students knew exactly what it was that they needed to work on in all areas of the curriculum. Setting goals as mathematicians is just as important as setting goals for work in the areas of reading and writing.
Below is the chart that I created and students used to sign up for upcoming small group math lessons with me. Group topics changed as the needs of the students evolved.

I purchased full-size sheets of dry erase boards at Home Depot and cut them down to 3'x4' pieces. I then took Duct tape and put it around the edges.
